장기요양인정의 신청자격
- 65세 이상의 노인 및 65세 미만으로 노인성 질병을 가진 자가 자격 대상입니다. (노인성질병 : 치매, 뇌혈관성질환, 파킨슨 병 등 보건복지부 장관이 정하여 고시한 질병). Those aged 65 or older and those under 65 with senile diseases are eligible. (Geriatric Disease: Diseases determined and publicly notified by the Minister of Health and Welfare, such as dementia, cerebrovascular disease, and Parkinson's disease).
장기요양인정의 신청(노인장기요양보험법 제 13조)
- 건강보험공단지사(노인장기요양보험지원센터)에서 접수합니다. The Health Insurance Corporation (Long-Term Care Insurance Support Center for the Elderly) applies.
- 신청인:본인 또는 대리인. applicant : self or agent.
(대리인:가족, 친족 또는 이해관계인, 사회복지전담공무원, 시장, 군수, 구청장이 지정하는 자                
대리신청시 대리인 본인임을 확인할 수 있는 신분증 제시, 다만 팩스, 우편접수경우 신분증 사본제출. Agent: Persons designated by family members, relatives or stakeholders, public officials exclusively in charge of social welfare, mayors, county  heads, or the head of the Gu; Presenting an identification card that identifies himself/herself at the time)

- 장기요양인정신청서 ( 공단지사 또는 홈페이지 https://www.longtermcare.or.kr )
의사소견서 제출기한(노인장기요양보험법 시행규칙 제2조~4조)
- 65세 이상 노인 : 등급판정위원회에 자료 제출 전까지. Senior citizens aged 65 or older: Before submitting data to the Rating Board.
- 65세 미만 중 노인성 질병을 가진자 : 신청서 제출시. Persons under 65 who have senile diseases : When submitting applications.
